Adding an Employer

Adding an employer in FormFire is easy. Just enter the basic information about the group, contact person, and a welcome letter will be sent out automatically. You may also assign the employer to a specific team in your agency.

Entering Employer Information

Adding a Medical Condition

FormFire uses an extremely advanced Medical Health Questionnaire to capture relevant medical history information. This example demonstrates how a single condition is entered.

eSignature Technology

FormFire's proprietary eSignature technology simulates a "wet" signature that can be printed directly on employee and employer applications.

This is an example of a medical form produced by FormFire. Using the complete set of data provided by the employee, any application can be created with just a few clicks.

Sample Additional Info Page

Wherever possible, FormFire will print all information on the carrier application. However, if addtional space is required, an underwriter-friendly and organized additional sheet is used.

Any data in FormFire can be exported to whatever format you require. Whether a generic census, or specific format required by your agency management system.
