Form Mapping

At the most basic level, for carriers who aren't ready to accept a data feed or utilize digital underwriting, FormFire can map employee medical information directly to carrier applications.

• Complete - FormFire's mapping ensures that all available data fits on the appliction. If there is any run-off data or information that won't fit in the space allotted, it is marked and placed on a separate sheet.
• Correct - Because the information was collected with FormFire's dynamic interview, the information sent to the carriers will include all relevant information required for underwriting the group, entered directly by the employee.
• Legible - Because the information is typed, underwriters will not have to decipher often illegible employee handwriting
• Organized - Any additional information is appended on an additional sheet following a format developed specifically for underwriters. It is often better organized that the carrier applications themselves.
• Carrier-specific - only information relevant to each carrier is sent. This means that not only are requested condition details sent, but only condition details that fall within the requested date range. For example, carriers that only wish to know about the past five years of employee medical history are only sent the past five years.
